Free or low-cost health insurance through Medicaid or Children's Health Insurance Program (CHIP) opens doors to healthy futures with:

  • Physical and speech therapy
  • Family planning
  • Counseling and more
You may qualify for children's free or low-cost health insurance if your adjusted monthly income fits the levels below.

 * CHIP costs include a monthly premium of $15 per child up to $45 maximum per family.

Income limits are valid through March 2010. 

Certain expenses can be deducted—families who earn more than this may still qualify. Please call CHAP to discuss your personal situation.

Use this application only to apply for free or low-cost medical coverage for a child under 19 years of age. This child must live in Washington State. Mail the completed, signed application with proof of income to:
