2009 Initiatives: Health Home
At the heart of the Washington Health Foundation (WHF) is a belief that big change is possible. To that end, WHF seeks to transform the health care delivery process by making “health care” a more personal endeavor. To do so, WHF believes that each Washingtonian should have a Health Home that includes:
1. A central resource for health and wellness information;
2. A health partner or provider advising on decisions and coordinating care;
3. A central collection point for individual health records; and
4. An individual health and wellness plan that is actively implemented.
In 2009-2010, WHF has made Health Homes a priority initiative of its Healthiest State in the Nation Campaign—the largest civic engagement project for health in Washington state history. It is also one of the Healthiest State Campaign’s specific policy recommendations for National Health Reform.
Partners in WHF’s Health Home Initiative include the Office of the Washington State Attorney General, Swedish Health Services and the Association of Washington Public Hospital Districts.
Resources
The work of the Health Home Initiative includes the production of a series of consumer-friendly guides that are specifically designed to help you become more engaged in your own health care. These downloadable materials are located under the My Health section of this website.
A special focus of this project has been 18-24 year old undergraduate students in Washington’s four year colleges and universities. The My Health section also includes featured information for this demographic.
